Summary:
GRANT FUNDED – Provides screenings and assessments to women and their families eligible for the healthy start program within patient homes. Assesses individual patient needs for resources to create care plans and arrange for coordination referrals for services that eliminate social  determinants  of health.

Grant funded positionsat Broward Health are often temporary or project-based, as they depend on the duration of the grant funding. Once the grant period ends or the allocated funds are exhausted, the position may be terminated unless additional funding is secured.

Education: Bachelor's degree in Social Work

Credentials: Bilingual preferred; Spanish or Creole
